Please state if there is a preference / or if the animal is particularly keen or averse to one gender

Behavioural

If yes, please describe in as much detail as possible
If yes please describe full circumstances
if yes, please describe full circumstances and how severe the bite was. If there have been several bite incidents please detail each one
if yes, please describe what the dog guards, what behaviours they exhibit, in as much detail as possible
If the animal is destructive or distressed when left alone for even short periods

Health

Please upload a photograph of the animal's vaccination card showing animal name, microchip number, and vaccination history
Please tell us what your animal will be able to bring with them to their new home. At minimum you should send them with a couple of weeks supply of their current food. Please detail whether you will also be sending them with their bed, toys, lead, collar, toys etc

If yes, this surrender form with this consent will be sent to your vet to apply for their medical history
Our decision whether to accept your pet is not reliant on a donation. A donation is not mandatory but does support Rescues in the work they do
Whilst we endeavour to maintain contact with previous owners, the animal will become the responsibility of the rescue, and eventually their new home and we cannot guarantee that ongoing updates will be available in all circumstances
